What is it?

General characteristics: International and chemical name: Aciclovir; 2-amino-9-[(2-hydroxyethoxy)methyl]-1,9-dihydro-6H-purin-6-one; Main physical and chemical properties: Homogeneous ointment of white color with a faint specific odor; Content: 1 g of ointment contains 0.025 g of acyclovir calculated on a 100% substance; Excipients: Proxanol 268, propylene glycol, polyethylene oxide 400, emulsifier #1. Pharmacotherapeutic group. Dermatological agents. Aciclovir. Pharmacological properties. Pharmacodynamics. Aciclovir is a deoxyguanidine purine nucleoside – an analog of a DNA component. It is particularly effective against herpes simplex and varicella viruses. The similarity in structure between acyclovir and deoxyguanidine allows acyclovir to interact with viral enzymes, leading to the cessation of viral replication. After acyclovir enters a herpes-infected cell, it is converted by thymidine kinase released by the virus into acyclovir monophosphate, which is then converted by host cell enzymes first into acyclovir diphosphate and then into the active form, acyclovir triphosphate, which selectively blocks viral DNA synthesis. Acyclovir triphosphate has practically no effect on the DNA replication of host (human) cells. In herpes, acyclovir prevents the development of new rash elements, reduces the possibility of skin dissemination and visceral complications, accelerates crust formation, and alleviates pain in the acute phase of shingles. Pharmacokinetics. When used topically, it is practically not absorbed into the systemic circulation and does not have a systemic effect. When the ointment is applied to the affected skin area, a therapeutic concentration of the drug is created, which is maintained at a constant level for at least 5 hours. Indications for use. Herpetic lesions of the skin and external mucous membranes (for herpes of the lips, external genitalia), caused by Herpes simplex virus types 1 and 2, Varicella zoster virus, primary and recurrent herpetic lesions in immunocompromised patients. Method of administration and dosage. For external use only! It is recommended to use rubber gloves or a finger cot to avoid spreading the infection. Treatment should be started at the first signs of herpes infection. The ointment is applied to the affected skin and external mucous membranes. Before applying the ointment, it is necessary to wash the affected surface with soap and dry it thoroughly. The ointment is applied in a thin layer calculated as follows (for adults): a 1.25 cm line of ointment is applied to 25 cm2 of the affected area 4-5 times a day and rubbed in gently for 1 minute. For children aged 3 months to 12 years, the ointment is applied in a thin layer calculated as 250 mg acyclovir/m2 of body surface area. The ointment is also applied to the areas adjacent to the affected surface. The ointment should be used regularly. If a dose is missed, it is not necessary to double the dose for subsequent applications. It is recommended to complete the full course of treatment – 10 days, even if the symptoms of the disease disappear earlier. If there is no effect with external use, it is recommended to prescribe the drug in tablet form for oral administration (the doctor makes the decision in each individual case). Side effects. During the first applications, the following may occur: a burning sensation, mild pain at the application site, itching. Inflammation may develop if it comes into contact with internal mucous membranes. Contraindications. Hypersensitivity to Herpevir, infectious diseases with similar symptoms but caused by other pathogens. The drug is not recommended for use during pregnancy and lactation (lactation is stopped during treatment). Children up to 3 months of age. Overdose. Prolonged use of the ointment in the application area may cause erythema, peeling, burning sensation, and burns, which disappear shortly after discontinuation of the drug. Special precautions. Herpevir should be used as soon as possible after the first symptoms of herpes appear. Affected areas should be protected from irritation. Sexual contact should not be made if one of the partners has symptoms of herpes. The ointment should not come into contact with the mucous membranes of the mouth, nose, eyes, or vagina. Interaction with other drugs. Probenecid slows down the excretion of the drug, prolonging its half-life. The interaction of Herpevir ointment with other locally applied drugs has not been studied. Storage conditions. Store in a dry place, inaccessible to children, at a temperature of 80°C-150°C. Shelf life. 3 years. Packaging. 5 g or 15 g in a tube, 1 tube in a box.
