What is it?

International Name - LIDOCAINE Active Substance: LIDOCAINE Clinical-Pharmacological Group: Local anesthetic, antiarrhythmic agent. Composition and Dosage Form: Solution for injection 10%: in ampoule 2 ml, in a pack of 5 pcs. 1 ml of 1 ampoule Lidocaine hydrochloride ............. 100 mg 200 mg Excipient: Water for injection. Indications: Primary prophylaxis of malignant ventricular arrhythmias, ventricular fibrillation and tachycardia in the early (hospitalization) phase of myocardial infarction. Treatment of ventricular tachycardia in the acute phase of myocardial infarction or before hospitalization of a patient with myocardial infarction, as well as ventricular tachycardia in other organic heart diseases. Secondary prophylaxis of ventricular tachycardia and/or fibrillation within 6-24 hours after the cessation of arrhythmia. Dosage and Administration: Diluted solution should be administered as an intravenous bolus or infusion. Intravenous bolus administration: Slow administration is necessary after 5-fold dilution in physiological saline (1 ml of diluted solution contains 20 mg of lidocaine). For adults, this solution should be administered at a dose of 1-2 mg/1 kg of body weight, with a maximum dose of 100 mg. The injection can be repeated in 5-10 minutes at a dose of 0.5-0.75 mg/1 kg of body weight, up to a total dose of 300 mg. Infusion: Used after bolus IV injection. One ampoule of 10% lidocaine solution should be diluted in 500 ml of Isodex or Ringer's infusion solution. Administer at a rate of 20-55 mcg/1 kg of body weight per minute (maximum 1-4 mg/min). For maintenance therapy, infusion can be continued at a rate of 1 mg/min as long as the patient's condition requires it. The maximum dose should not exceed 4.5 mg per kg of body weight, i.e., a total of 300 mg per hour. In case of heart failure or impaired liver function, the dose should be reduced by 40%. Since the clearance of the drug from the plasma is reduced in elderly patients with heart failure, a dose reduction is necessary in this case. Lidocaine administration requires constant medical supervision with ECG monitoring. Side Effects: Side effects occur in 5-10% of patients and depend on the dose of the drug. Central Nervous System: Dizziness, numbness of the oral mucosa and tongue, drowsiness, tinnitus, with higher doses - excitation, euphoria, disorientation. Convulsions, tremor, loss of consciousness, respiratory depression. Mild side effects resolve after discontinuation of the drug. In case of tremor and tonic-clonic seizures, parenteral administration of diazepam or short-acting barbiturates is necessary. Gastrointestinal Tract: Nausea, vomiting (frequent). Cardiovascular System: Rarely - hypotension, bradycardia, conduction disturbances, possible allergic reactions (hypersensitivity) to lidocaine, such as skin rash and swelling. Anaphylactoid reactions are very rare. Pregnancy and Lactation: Although reproductive studies in rats with doses 6.6 times higher than human doses did not reveal adverse effects on the fetus, generalization of these results to humans is not permissible. Adequate controlled studies in pregnant women have not been conducted. Lidocaine crosses the placental barrier, so during pregnancy (especially in the first trimester), it should be prescribed only after careful consideration of all risks and benefits. Lidocaine is excreted in breast milk in small amounts, which may be risky for the newborn. Therefore, caution should be exercised when administering the drug to nursing mothers. Contraindications: * Hypersensitivity to lidocaine or a history of seizures after administration of this drug; * Atrioventricular block of II-III degree; * Severe bradycardia; * Adams-Stokes syndrome; * Left ventricular dysfunction
