What is it?
International Name - Sildenafil Citrate Group: Erectile Dysfunction Medications Indications: Erectile dysfunction, characterized by the inability to achieve or maintain an erection sufficient for sexual intercourse. Dosage and Method of Administration: The preparation is taken orally. For most patients, the recommended dose is 50 mg, taken approximately 1 hour before sexual activity if needed. Depending on the effectiveness and tolerability of the dose, it can be increased to 1000 mg or reduced to 25 mg. Maximum recommended frequency – once a day. The following factors influence the increase in sildenafil levels in blood plasma (AUC): age over 65 years (40% increase in AUC), liver dysfunction (e.g., liver cirrhosis, 80%), severe renal insufficiency (creatinine clearance < 30 ml/min, 100%), concomitant use of cytochrome P450 3A4 inhibitors (e.g., ketoconazole, itraconazole, erythromycin, 182%, saquinavir 210%). This can contribute to both increased efficacy of the preparation and the development of side effects. It is necessary to consider the advisability of prescribing the preparation at a dose of 25 mg/day in patients of this group. In studies of healthy, non-HIV-infected volunteers, ritonavir strongly increases systemic sildenafil levels in plasma. Based on this pharmacokinetic data, it is not recommended to administer more than a single maximum dose of 25 mg within 48 hours. Contraindications: Hypersensitivity to the components of the preparation. *The use of the preparation is contraindicated in patients who are taking nitric oxide donors, organic nitrates, or nitrates in any form, continuously or intermittently, as sildenafil potentiates the hypotensive effect of nitrates used continuously or in emergency cases. Side Effects: Side effects are transient, mild to moderate in severity. The nature of side effects in dose-finding studies, as such studies better reflect the recommended regimen of use, was compared to studies with fixed doses. In placebo-controlled clinical trials with recommended fixed doses of sildenafil, the following side effects were observed: headache, fever, dyspepsia, nasal congestion, eye pain, eye redness, diarrhea, dizziness, rash.
