- ESLOTIN 5MG 10 TABLETS International Nonproprietary Name Desloratadine Dosage Form Film-coated tablets Description: Round white tablets with "DLT 5" engraving on one side and a dividing score. Composition: Each film-coated tablet contains Active substance: Desloratadine 5 mg Excipients: Lactose, corn starch, magnesium stearate Pharmacotherapeutic group: Antihistamines for systemic use. H1-histamine receptor blocker. Pharmacological properties Pharmacodynamics: Eslotin has antihistamine, antiallergic, and anti-inflammatory effects, and does not cause drowsiness. When taken orally, desloratadine (the primary active metabolite of loratadine) selectively blocks peripheral H1-histamine receptors. It suppresses the cascade of cytostatic reactions: the release of pro-inflammatory cytokines, including interleukin-4 (IL-4), interleukin-6 (IL-6), interleukin-8 (IL-8), interleukin-13 (IL-13), as well as RANTES type pro-inflammatory chemokines, production of superoxide anions by activated polymorphonuclear neutrophils, eosinophil adhesion and chemotaxis, expression of adhesion molecules, including P-selectin, release of histamine induced by IgE from D2 prostaglandin and C4 leukotriene. Pharmacokinetics: The drug is well absorbed from the gastrointestinal tract, with maximum plasma concentration reached approximately 3 hours after administration. The half-life is approximately 27 hours. Bioavailability is proportional to the dose taken (in the dose range of 5 to 20 mg). It does not cross the blood-brain barrier, crosses the placental barrier and is excreted in breast milk. Indications: - Seasonal pollinosis and perennial allergic rhinitis (to relieve or alleviate sneezing, nasal discharge, nasal congestion and itching, watery eyes, eye itching and redness); - Chronic idiopathic urticaria (to reduce and eliminate skin itching and rash). Contraindications: - Hypersensitivity to the components of the drug; - Pregnancy and lactation. Side effects: Increased fatigue, dry mouth, headache. Very rarely – tachycardia, palpitations, increased activity of liver enzymes, increased bilirubin concentration, allergic reactions, including anaphylaxis and rash. Dosage and administration: Eslotin is taken orally, regardless of food intake, at a dose of 5 mg once daily. The tablet should be swallowed whole, without chewing, with water. Overdose: Symptoms: Intensification of side effects. Treatment: Gastric lavage followed by administration of activated charcoal, if necessary – symptomatic therapy. Hemodialysis is ineffective. The effectiveness of peritoneal dialysis has not been established. Interactions with other drugs: Clinically significant changes in desloratadine plasma concentration were not observed during multiple co-administration of desloratadine with ketoconazole, erythromycin, azithromycin, fluoxetine, cimetidine. Administration of the drug with food does not affect the distribution of desloratadine in the body. Desloratadine does not enhance the effect of alcohol on the central nervous system. Special instructions: The drug should be used with caution in patients with severe renal insufficiency. Use in pediatrics: Can be used as prescribed by a doctor. Effect on ability to drive and operate machinery: Eslotin does not affect the ability to drive vehicles and perform tasks that require speed of psychomotor reactions. Состав каждая таблетка, покрытая оболочкой, содержит активное вещество: дезлоратадин 5 мг вспомогательные вещества: лактоза, кукурузный крахмал, магния стеарат Фармакотерапевтическая группа антигистаминные средства для системного применения. Блокатор H1-гистаминовых рецепторов. Фармакологические свойства Фармакодинамика Эслотин обладает антигистаминным, противоаллергическим и противовоспалительным действием, не вызывает сонливости. При приеме внутрь дезлоратадин (первичный активный метаболит лоратадина) селективно блокирует периферические H1-гистаминовые рецепторы. Подавляет каскад цитостатических реакций: высвобождение провоспалительных цитокинов, включая интерлейкин-4 (ИЛ-4), интерлейкин-6 (ИЛ-6), интерлейкин-8 (ИЛ-8), интерлейкин-13 (ИЛ-13), а также провоспалительных хемокинов типа RANTES, продукцию супероксидных анионов активированными полиморфноядерными нейтрофилами, адгезию и хемотаксис эозинофилов, экспрессию молекул адгезии, включая P-селектин, высвобождение гистамина, обусловленное IgE, D2 простагландина и C4 лейкотриена. Биодоступность пропорциональна принятой дозе (в диапазоне доз от 5 до 20 мг). Не проходит гематоэнцефалический барьер, преодолевает плацентарный барьер и выделяется с грудным молоком. Показания к применению - сезонный поллиноз и круглогодичный аллергический ринит (для снятия или облегчения чихания, выделения слизи из носа, заложенности носа и зуда, слезотечения, зуда и покраснения глаз); - хроническая идиопатическая крапивница (для уменьшения и исчезновения зуда и сыпи на коже). Противопоказания - гиперчувствительность к компонентам препарата; - беременность и период лактации. Побочное действие повышенная утомляемость, сухость во рту, головная боль. Очень редко – тахикардия, ощущение сердцебиения, повышение активности печеночных ферментов, повышение концентрации билирубина, аллергические реакции, включая анафилаксию и сыпь. Способ применения и дозы Эслотин принимается внутрь, независимо от режима питания, в дозе 5 мг 1 раз в сутки. Таблетку следует проглатывать целиком, не разжевывая, запивая водой. Передозировка Симптомы: усиление проявления побочных реакций. Лечение: промывание желудка с последующим приемом активированного угля, при необходимости – симптоматическая терапия. Гемодиализ неэффективен. Эффективность перитонеального диализа не установлена. Взаимодействие с другими лекарственными средствами При многократном совместном применении дезлоратадина с кетоконазолом, эритромицином, азитромицином, флуоксетином, циметидином при изучении лекарственных взаимодействий не выявлено клинически значимых изменений концентрации дезлоратадина в плазме. Прием препарата с пищей не влияет на распределение дезлоратадина в организме. Дезлоратадин не усиливает действие алкоголя на центральную нервную систему. Особые указания Препарат с осторожностью применяется у пациентов с острой почечной недостаточностью. Применение препарата в педиатрии возможно по назначению врача. Влияние на способность управлять автомобилем и другими механизмами Эслотин не влияет на способность управлять автомобилем и выполнять работы, требующие быстроты психомоторных реакций. Форма выпуска 10 таблеток, покрытых оболочкой, в блистере. 1 блистер в картонной коробке вместе с инструкцией по применению. Условия хранения Хранить при температуре не выше 25ºC, в недоступном для детей месте. Срок годности 3 года с даты производства. Препарат не применяется после истечения срока годности.
