LOZATENZI RM TABLET 100MG+25MG COMPOSITION: Each film-coated tablet contains: Losartan Potassium USP - 100 mg Hydrochlorothiazide USP - 25 mg Excipients - q.s. Color: Titanium Dioxide BP INDICATION: Losartan Potassium / Hydrochlorothiazide is used for the treatment of essential hypertension in patients whose blood pressure is not adequately controlled by losartan alone or hydrochlorothiazide alone. DOSAGE: Hypertension Losartan and Hydrochlorothiazide are not intended for use as initial therapy but are indicated for the treatment of patients whose blood pressure is not adequately controlled by losartan potassium alone or hydrochlorothiazide alone. Dose titration of the individual components (losartan and hydrochlorothiazide) is recommended. A direct switch from monotherapy to a fixed-dose combination may be considered in patients whose blood pressure is not adequately controlled, based on clinical appropriateness. The standard maintenance dose is one tablet of Losartan Potassium/Hydrochlorothiazide 50mg/12.5mg (Losartan 50 mg/HCTZ 12.5 mg) once daily. For patients who are not adequately responding to Losartan Potassium/Hydrochlorothiazide 50mg/12.5mg, the dose may be increased and administered as a maximum of two tablets of Losartan Potassium/Hydrochlorothiazide 50mg/12.5mg once daily or one tablet of Losartan Potassium/Hydrochlorothiazide 100mg/25mg (Losartan 100 mg/HCTZ 25 mg) once daily. The antihypertensive effect is usually achieved within three to four weeks of the initiation of therapy. ADMINISTRATION: Losartan Potassium/Hydrochlorothiazide may be administered with other antihypertensive agents. Losartan Potassium/Hydrochlorothiazide tablets should be swallowed with a glass of water. Losartan Potassium/Hydrochlorothiazide tablets may be taken with or without food. CONTRAINDICATIONS: Hypersensitivity to losartan, sulfonamide derivatives (such as hydrochlorothiazide) or to any of the excipients Refractory hypokalemia or hypercalcemia Severe hepatic impairment; cholestasis and obstructive biliary diseases Refractory hyponatremia Symptomatic hyperuricemia/gout 2nd and 3rd trimester of pregnancy Severe renal impairment (e.g., creatinine clearance <30 mL/min) Anuria Concomitant use of Losartan Potassium/Hydrochlorothiazide with aliskiren-containing medicinal products is contraindicated in patients with diabetes mellitus or renal impairment (GFR < 60 mL/min/1.73 m2). STORAGE: Store at a temperature not exceeding 25°C. Keep out of reach of children.
