What is it?

Composition: 1 film-coated tablet Uperio 24 mg-26 mg, as active substance, contains: sacubitril and valsartan sodium salt hydrate complex 56.551 mg. Excipients: microcrystalline cellulose, low-substituted hydroxypropylcellulose, crospovidone, magnesium stearate (vegetable origin), talc, and colloidal silicon dioxide. Film coating excipients: hypromellose, titanium dioxide (E171), macrogol 4000, talc, iron red oxide (E172), iron black oxide (E172). Pharmacotherapeutic group: Drugs affecting the renin-angiotensin system. Angiotensin II receptor antagonists, other combinations. Valsartan and sacubitril. Indications: Treatment of heart failure in patients with systolic dysfunction. Dosage and administration: The preparation is for oral administration, independent of food intake. The target dose is 97 mg-103 mg twice daily. The recommended starting dose of Uperio is 49 mg-51 mg twice daily. A starting dose of Uperio 24 mg-26 mg twice daily is recommended for patients who are not currently taking an angiotensin-converting enzyme inhibitor or an angiotensin II receptor blocker, and a similar dose may be recommended for patients who have previously taken low doses of ACE inhibitors. The dose of Uperio should be doubled every 2-4 weeks until the target dose of Uperio 97 mg-103 mg twice daily is reached, depending on tolerability. Since there is a risk of developing angioedema when using Uperio with an ACE inhibitor, Uperio should not be initiated less than 36 hours after stopping ACE inhibitor therapy. In patients with severe renal impairment, the recommended dose of Uperio is 24 mg-26 mg twice daily. Dose adjustment is not required in patients with mild renal impairment. In patients with moderate hepatic impairment, the recommended dose of the preparation is 24 mg-26 mg twice daily. Dose adjustment is not required in patients with mild hepatic impairment. The safety and efficacy of the preparation in children and adolescents under 18 years of age have not been established. Dose adjustment is not required in patients over 65 years of age. Side effects: Hyperkalemia, hypokalemia, dizziness, postural dizziness, headache, vertigo, hypotension, syncope, orthostatic hypotension, cough, diarrhea, nausea, angioedema, renal impairment, renal failure, increased fatigue, general weakness. Contraindications: Hypersensitivity to the active substances, sacubitril, valsartan, or any of the excipients. Concomitant use with ACE inhibitors. Uperio must not be used less than 36 hours after discontinuation of therapy with an ACE inhibitor. Angioedema in the history associated with previous use of ACE or ARB inhibitors. Concomitant use with aliskiren in patients with type 2 diabetes mellitus. Pregnancy. Interactions with other drugs: Concomitant use with ACE inhibitors and aliskiren is contraindicated. Potential interactions for which concomitant use is not recommended: Angiotensin receptor blockers. Use with caution in combination with statins, sildenafil. Potential interactions requiring consideration: Concomitant use of potassium-sparing diuretics, such as triamterene, amiloride, mineralocorticoid antagonists, such as spironolactone, eplerenone, potassium or salt substitutes containing potassium, may lead to an increase in serum potassium and creatinine levels.
