What is it?

International Nonproprietary Name - Natrii hydrocarbonas Pharmacological group: Electrolyte and acid-base balance correcting agents Composition: Active substance: Sodium bicarbonate; 1 ml of solution contains 42 mg of sodium bicarbonate; Excipients: Disodium edetate, Carbon dioxide, Water for injection. Dosage form. Solution for infusion. Main physical and chemical properties: Clear, colorless liquid. Ionic composition per 1 L of preparation: Sodium ion - 0.5 mol, Bicarbonate ion - 0.5 mol. Theoretical osmolarity - 1000 mOsm/L. pH 7.0-8.5. Indications. Uncompensated metabolic acidosis in intoxications of various etiologies, complicated postoperative period, extensive and/or deep burns, shock, diabetic coma, prolonged diarrhea, persistent vomiting, acute massive hemorrhage, severe liver and kidney damage, prolonged fever, severe hypoxia in newborns. A reduction in blood pH below 7.2 is an absolute indication. Contraindications. Metabolic or respiratory alkalosis, hypokalemia, hypernatremia. Special precautions for use. Blood pH and acid-base balance must be monitored. In patients with heart or kidney diseases, heart failure and edema may develop. In case of very rapid correction of acidosis, especially with impaired lung ventilation, rapid release of CO2 may worsen cerebral acidosis. Small amounts of Soda-Buffer, administered with other infusion solutions with acidic pH, act as neutralizing agents and prevent the occurrence of post-infusion phlebitis (glucose solutions of various concentrations, Ringer's solution, sodium chloride solution, Ciprofloxacin and some other fluoroquinolones). Use during pregnancy and lactation. It is not known whether sodium bicarbonate causes adverse effects on the fetus when administered to pregnant women, nor whether it affects reproductive function. Therefore, this drug should be prescribed to pregnant women only if the expected benefit to the mother outweighs the potential risk to the fetus. Use of the drug during lactation is possible only in vital indications. Method of administration and dosage. Administered to adults and children intravenously by drip infusion at a rate of 1.5 mmol/kg per hour (3 ml of 4.2% Soda-Buffer /kg per hour). In newborns, administered at a dose of 1-2 ml per 1 kg of body weight, once, under control of acid-base and water and electrolyte balance. When correcting metabolic acidosis, the dosage is determined taking into account the level of acid-base balance disorder. The dose is calculated based on blood gas parameters using the following formula: Volume of 0.5 mmol buffered sodium bicarbonate in 4.2% ml = Base deficit (- BE) X kg of body weight X 0.3 X 2 (0.3 factor corresponds to the proportion of extracellular fluid relative to total fluid). Maximum dose of the preparation in adults - 300 ml (400 ml in case of overweight) per day; in children - from 100 to 200 ml according to body weight. Children. Can be used from birth.
